The termination of membership of the deceased person takes place as of the date of death once the death has been reported.   1. Please notify us of the death using the appropriate form<\/a>.<\/li>
  2. For the review of entitlement to survivors\u2019 benefits, we would appreciate it if you could provide us \u2013 if known \u2013 with a contact person from the deceased person\u2019s family.<\/li>
