The termination of membership of the deceased person takes place as of the date of death once the death has been reported.
Procedure
- Please notify us of the death using the appropriate form.
- To review entitlement to survivors’ benefits, please provide the name of a contact person from the deceased person’s family, if known.
- If no contact person is provided, we will request the necessary documents via the employer.
